New Life Christian Academy is blessed to have a full time registered nurse, Ms. Linda Thomas. The school nurse can be reached at 210-679-6006. School Physicals Required for All Students:
All students at New Life Christian Academy are required to have an annual school physical. The form is available by clicking the link below and may be used for the free physicals or brought to your regular physician. Contact Linda Thomas, School Nurse, for questions: Call 210.679.6006.

Health Form Requirements for All Students:
Forms for the School Nurse are posted on the Web. HS001 and HS002 are Mandatory.  HS003 is for students who will require Inhalers, Epi-pens and/or Prescription medication during school hours. This form must be signed by a Physician. Incoming 9th graders should have received their forms during Orientation. The completed health forms are necessary before the first day of school and will be stored by the School Nurse.
